Position Summary

The Advanced Practice Provider delivers comprehensive primary and preventive healthcare services to patients of all ages, with an emphasis on whole-person care and evidence-based treatment. The provider will diagnose and treat acute and chronic conditions, deliver MAT services, and collaborate with an interdisciplinary care team to ensure optimal patient outcomes.


Key Responsibilities

  • Provide comprehensive primary and preventive care to diverse patient populations
  • Diagnose and manage acute illnesses and chronic conditions
  • Deliver Medication-Assisted Treatment (MAT) for patients with substance use disorders
  • Order, perform, and interpret diagnostic testing (labs, imaging, studies)
  • Prescribe medications and evidence-based treatment plans
  • Counsel patients on disease prevention, health promotion, and self-management
  • Maintain accurate and timely documentation in the Electronic Medical Record (EMR)
  • Develop and implement individualized treatment plans following clinical protocols
  • Refer patients to specialists and community resources as appropriate
  • Perform in-office procedures including:
    • IUD and Nexplanon placement
    • Pap smears and biopsies
    • Joint injections
    • Nail removals
    • Suturing
    • Wart cryotherapy
    • Incision & drainage

Qualifications

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program
  • Active South Carolina APRN or Physician Assistant license
  • Current Basic Life Support (BLS) certification
  • Experience providing Medication-Assisted Treatment (MAT)
  • Minimum of 3 years clinical experience preferred
  • Commitment to serving underserved and vulnerable populations
