The Advanced Practice Provider works to support the mission and vision of DAP Health by providing excellent patient-centered care and services within a dynamic team environment. This role will provide direct healthcare services to patients under the direction and responsibility of our team of Physicians.
Assess and interpret medical and psycho-social histories; perform physical examinations (including gynecological and breast examination as appropriate; identify problems; formulate a plan of treatment; provide education and referral (as indicated); and evaluate the effectiveness of the interventions prescribed
Independently assess and treat patients as clinically indicated. Consult with or refer clients with abnormal conditions or other medical, psychological, or social problems to the collaborating physician or another appropriate referral course when indicated
Perform or order laboratory tests and interpret the results as part of health management. Report communicable diseases in a timely manner to the appropriate Health Department as per health center protocol
Order medications
Follow established treatment guidelines as indicated by national guidelines or Health Center protocols
Meet clinical quality performance measures (UDS and HEDIS measures)
Conduct health teaching and counseling during the medical encounter, as appropriate, enabling the client to make informed choices about health and treatment options
Complete the client's clinical record documentation in a timely manner specific to the applicable policies and procedures of the health center
Participate in quality improvement/assurance activities
Participate in continuing medical education activities to enhance professionalism and to ensure quality of care
Provide on-call coverage as scheduled
Work collaboratively with all members of the health center to build a supportive team
Apply infection control measures according to Health Center Policies and Procedures
Maintain professionalism and adhere to client confidentiality
Perform other duties as assigned
Triage new and existing patients to determine acuity of immediate complaint; assess, diagnose, and treat
Administer all diagnostic and treatment protocols
Provide quality care to the patients within the scope of practice outlined by state or federal law and understand the organization's commitment to provide high quality patient care
Perform physical examinations and/or preventive health measures to assess and evaluate patient's clinical problems and health care needs
Refer patient to other available services/specialist to meet needs where appropriate Examine patients, formulate differential diagnostic plans, and define and order required diagnostic testing
Interpret examination findings and test results and implement treatment plans
Instruct patients in their proper care and treatment alternatives and instruct/supervise medical support staff as needed
Remain at the clinic site until all patients have been seen or treated, unless other treatment/care plans have been established
Follow established guidelines when referring patients out for needed specialty or diagnostic care
Communicate clearly and effectively with clients and other medical personnel Maintain and update patient charts, records, and information in a timely manner
Assist in internal quality control audits
Participate in peer review
Supervise medical assistants and lab personnel in the area of specific support services for patient care delivery
Attend internal and external meetings, workshops, and conferences, and participate in task forces or committees as necessary and/or required
Perform all duties and services in full compliance with standards of care as well as all policies and procedures
Provide exceptional customer service
Participate in Quality Improvement (Q') Initiatives
Comply with Attendance & Punctuality Policy
Work closely with CMO, physicians, and other personnel in order to develop and maintain clear and concise treatment protocols
Access patient-protected health information as required
Perform other duties as assigned by your supervisor
