Position Summary:
A Nurse Practitioner (NP) or Physician Assistant (PA) works under the supervision / delegation of the Chief Medical Officer (CMO), individual supervising physicians, Director of Advanced Practitioners, and APP Regional Managers.
Shifts and Hours: Full-time is minimum 12 shifts/month; PRN is minimum 2 shifts/month (additional shifts can be picked up)
- Hours for Clinical Staff: Monday - Saturday: 7:30am - 8:00pm and Sundays 11:30am - 8:00pm
FT Status: Minimum 12 shifts/month to include 2 Saturdays + 2 Sundays
- 1 On-call shift / month (must answer between 6am-3pm)
- Could be potentially dispatched to any DFW Clinic (not Wichita Falls)
- Salaried position - 100k-125k/year; based on 139 hours per month - Depends on Experience
- Based on approximately 139 hours / month
- Monthly pay period
- Not eligible for OT; extra shifts will be paid at an hourly rate in alignment with your normal salary rate (est. $60-75/hr)
- Ability to pick up extra shifts (at other clinics if available)
- Schedules made 60+ days in advance
- Performance evaluations within first 60-90 days
- PRN is hourly pay ($60-75/hr) on a biweekly pay schedule
- NOTE: PRN is not eligible for benefits but ARE eligible to participate in 401k plan AFTER 6 months of employment
- CLINICS CLOSED ON:
- Thanksgiving Day + Christmas Day + Easter + New Year’s Day + 4th of July
Required Qualifications:
- Minimum 1 year as a Provider
- Urgent Care experience highly preferred
- Ability to handle seeing 25-40 patients a day
- Unencumbered license(s) to practice as a Nurse Practitioner or Physician’s Assistant in Texas
- Current BLS certification, DEA license and any other certifications, board certifications, licenses, or regulatory mandates by the state to follow state laws
- Signed Delegation Protocol and Collaborative Practice Act Agreement
- It is the sole responsibility of the Mid-Level Medical Practitioner to maintain all certifications and registrations necessary as outlined by the state and federal government. Failure to do so may result in termination, as these are necessary for employment with CommunityMed.
Benefits: Eligible on 1st day of the month AFTER 30 days of FT employment

- Board certification (AANP or ANCC for NP's /or TAPA /or NCCPA for PA Certification)

- State license (PA or NP)

- DEA certificate or DEA number, issue date, expiration date and zip code of last registration
- BLS / ACLS certificate (Need within first 30 days) (Must be AHA compliant)
- DOT certificate (DOT from the National Board of Medical Examiners within 60 days of employment)
- TSCA application (Can be pulled from CAQH)
- Highest Level of Diploma / Education certificate
- Resume / CV
Employee provides /replies with the following information:
- NPI number
- PECOS / NPPES username and password
- CAQH username and password
Employee completes or attests immunizations:
- Proof of Hep B (Hepatitis B) (Required 3 series)
- Proof of Tdap (Tetanus, Diphtheria & Pertussis) (Required every 10 years)
- Proof of MMR (Measles, Mumps, Rubella)
- Proof of TB (Tuberculosis) (Skin test)
- Proof of Influenza Vaccine (Required yearly)
Employee reads and signs the following agreements:
- CommunityMed- APP Employment Agreement
- CommunityMed- Delegation Protocol and Collaboration Agreement (Prescriptive and Supervising Authority)
- CommunityMed- Five Rights
CMed completes the following actions:
- Malpractice (HR adds employee to policy)
