$97,000-$104,000 + full benefits (health, dental, vision, 401(k), paid holidays, and PTO)
Job Summary
Our client, a nonprofit community health organization, is seeking a Nurse Practitioner to provide comprehensive, patient-centered primary care services in a clinic setting. This role focuses on prevention, chronic disease management, and improving access to care for underserved populations.
Job Responsibilities:
Conduct patient assessments, diagnoses, and treatment planning Manage acute and chronic conditions using evidence-based practices Prescribe medications and order diagnostic testing Provide patient education on wellness and disease prevention Collaborate with a multidisciplinary care team Maintain accurate and timely documentation in the EHR Support quality initiatives and population health programs
Qualifications:
Master’s degree in Nursing (NP specialty) Active Florida NP and RN licenses Minimum 3+ years of clinical experience (NP and/or RN) Experience in primary care, community health, or similar settings preferred AAHIV credential preferred Strong communication and patient engagement skills
