AFHA CASE MANAGER

Job Summary

Monitors family homes to ensure quality of care, client satisfaction and compliance with regulations. Provides support and resources to Family Home Providers caring for individuals with developmental disabilities in their homes.

What You'll Be Doing: Tasks, Duties, and Responsibilities

  • Completes home visits to ensure quality of care, client satisfaction, and compliance with regulations.
  • Facilitates ISP meetings and discussions about clients' goals with clients and members of the support team.
  • Prepares written plans detailing client goals and areas where support is needed, and updates as needed.
  • Provides training and guidance to families regarding appropriate support to meet client goals and care needs.
  • Maintains charts according to specifications in home and office locations.
  • Communicates pertinent information to all members of the support team.
  • Provides on-call support to families outside of regular business hours, as needed.
  • Tracks and ensures compliance with ongoing requirements for Family Home Providers.

Qualifications

Education & Experience:

Must meet one of the following:

  • Bachelors Degree from an accredited four-year college or university with degree in psychology, social work, sociology, or a related field, OR
  • College coursework in the fields of psychology, social work, sociology, or a related field, AND one (1) year of applicable job experience in social services, OR
  • High school diploma or GED and a minimum of (2) years of applicable job experience in social services

Technical/Functional Skills:

  • Ability to read, write, speak, and understand English.
  • Proficient on office equipment including computer, printer, and copy machines.

Competencies:

  • Ability to form positive, supportive, and professional working relationships.
  • Excellent customer service skills with ability to address needs in a prompt, positive, and timely manner.
  • Ability to work in team environments and understand multiple perspectives.
  • Excellent communication and conflict-resolution skills with ability to directly address areas of concern.
  • Highly organized.
  • Meets deadlines and completes assignments in a timely manner.

Minimum Requirements:

  • Must be at least 18 years of age at the time of hire.
  • Must receive criminal record clearance from DOJ.
  • Must pass a pre-employment TB test.
  • Must have valid California Driver's license, safe driving record, and have been licensed to drive at least 2 years.
  • Must have a safe and reliable vehicle, and maintain current and adequate auto insurance.
  • Must obtain CPR/FA certification if not currently certified.
  • Must be fully vaccinated for COVID-19 (including required booster shots) or have received an approved exemption from HGH by date of hire.

Physical Demands/Environment:

  • While this job is mostly sedentary, the employee may be required to perform physical activity including walking, standing, sitting, reaching, carrying, lifting, bending, twisting, stooping and kneeling.
  • The employee may be required to lift office equipment up to 10 lbs.

We support children and adults in San Diego and Imperial Counties with intellectual and developmental disabilities, such as Down syndrome, epilepsy, autism, and cerebral palsy. We provide a full continuum of care including residential care in specialized group homes, independent living support, respite care, and adult foster home services. Additionally, we provide early childhood assessments and services, tailored day support, Life Planning workshops, county-wide transportation services, and on-site activities at our resource center.
