Qualifications:

Registered Nurse from an accredited program currently licensed and in good standing with the State Board of Nursing in the state of licensure. Minimum one year recent experience in an acute-care hospital or Home Health environment; previous experience as a Hospice RN preferred. CHPN certification is strongly encouraged within 3 years of employment.

Responsibilities:

Under the physician’s orders, uses the nursing process and nurse case management strategies to coordinate care and provide nursing services for hospice patients and their families in the home, long term care facility, assisted living facility, or hospital. This position reports to the Director of Inpatient and Program Development or designee.

Primary tasks and duties:

  • Promotes and practices the hospice mission and core values, and follows all policies and procedures of Infinity Hospice Care
  • Visits patients and provides clinical, physical, educational and emotional support after-hours as directed by the Triage team
  • Under the direction of a licensed physician, performs patient assessment, manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care
  • Utilizes nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit
  • Attends patient deaths, provides bereavement support to the patient caregiver/family, as indicated and performs post mortem care
  • Respond to and triages all after hours visits, admissions, referrals, GIP hospital patients.
  • Explains Medicare Hospice Benefit, obtains signatures on legal consent documents including
    informed consent, as requested
  • Provides accurate and timely documentation of patient services within EMR to reflect the
    individualized plan of care and outcomes of interventions
  • Documents and submits all visits in daily Call Log at end of shift via EMR
  • Reviews and updates the Plan of Care, physician’s orders, MAR, pharmacy profile and other
    records to ensure patient/family needs are met
  • Supervises LPNs and paraprofessionals providing services to the patient according to
    regulations
  • Recognizes and maintains professional boundaries in relationships with patient/family/caregiver
  • Demonstrates knowledge of and adheres to the state Nurse Practice Act as well as legal,
    regulatory, and accreditation requirements
  • Remains awake and alert during entire shift; demonstrates responsibility in work attendance
  • Respond to all calls/notifications within 15 minutes, per On Call Protocol

Every incidental duty connected with the After Hours RN-Field position cannot be specified in the job description and the colleague, at the discretion of the Director of Inpatient and Program Development, may be required to perform duties that are not included in this job description.


Knowledge, Skills and Abilities Required:

  • Possess strong patient observation, assessment, clinical and documentation skills
  • Self-starter, dependable with excellent communication, collaboration and organizational skills
  • Effective and working knowledge of computers and clinical information systems and typing
    30wpm
  • Able to work flexible hours (evenings, weekends and holidays) to meet identified organizational
    needs
  • Able to respond effectively to meet the treatment needs of chronically, acutely and terminally
    ill patients and their caregivers
  • Speak, write, read and comprehend English. Bilingual (English/Spanish) a plus
  • Possess reliable transportation, a valid driver’s license and current auto insurance

Requirements:

Education and experience:

  • Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice.
  • Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Licensure: Current State license as a Registered Nurse; Current driver’s license
  • Skills: Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations. Good interpersonal skills

Physical Requirements:

  • Environmental and Working Conditions: Works in patient homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency calls.
  • Physical and Mental Effort: Prolonged standing and walking required, with ability to lift up to 50lbs and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient/family individualized psychosocial needs. Requires eye-hand coordination and manual dexterity.
  • Ability to communicate with patients, families, physicians, co-workers, and visitors to be able to exchange accurate information regarding patient condition and health status. Ability to exchange and express information by means of language and communicate information effectively.
  • Ability to hear the nature of sounds. Ability to give full attention to what other people are saying, take time to understand the points being made, ask questions as appropriate, and not interrupt at inappropriate times.
  • Ability to make independent judgments and decisions. Ability to use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.
  • Ability to view tablet, Electronic Health Record, and review, assess, record or type data quickly and accurately.
  • Ability to make independent judgments and decisions is required.
  • Ability to determine resources needed to provide quality patient care.
  • Ability to travel to patient homes and office/support center locations as needed to deliver care or for education purposes.
