The Ambulatory Care Clinical Pharmacist provides evidence-based pharmaceutical care to patients in the ambulatory clinical setting. The delivery of care by the Ambulatory Care Clinical Pharmacist includes, but is not limited to: designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing best evidence and most effective and most economical medication treatments; helping to achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams in the ambulatory clinical setting. When appropriate, pharmacists who provide ambulatory care services may perform patient assessments, participate in collaborative practice agreements to provide medication management, provide collaborative drug therapy management, order, interpret, monitor medication therapy-related tests, coordinate care and other health services for wellness and prevention of disease, provide education to patients and caregivers, and document care plans in the medical record.

As an Ambulatory Care Clinical Pharmacist , you’ll collaborate with a large multidisciplinary team to deliver evidence-based medication management, improve access, and educate patients—helping them breathe easier and live better.

Your Impact
Partner with Providers to manage complex pulmonary conditions, including interstitial lung disease, idiopathic pulmonary fibrosis, sarcoidosis, COPD, and asthma.
Guide Patients through counseling, monitoring, and follow-up for specialty and non-specialty medications.
Optimize Access by navigating medication approvals and affordability solutions.
Serve as a Resource for medication information, adverse event management, and dose adjustments.

Educate & Inspire : Precept APPE students and residents (PGY1, PGY2 Ambulatory Care), lead projects, and deliver lectures/in-services.

Minimum Qualifications
Education: Bachelor’s in Pharmacy with experience or Pharm.D. from an ACPE accredited College of Pharmacy. Degree required.
Experience: PGY-1 residency required or equivalent clinical experience.
Licensure/Certifications: Commonwealth of Virginia State Board of Pharmacy License required. American Heart Association (AHA) Health Care Provider BLS certification required. Board Certification or equivalent certification specific to clinical area highly encouraged.

Preferred Qualifications
Education: Pharm.D. from an ACPE accredited college of Pharmacy.
Experience: Postgraduate Year Two (PGY2) Pharmacy Residency, or an equivalent amount of relevant work experience
Licensure/Certifications: Credentialed as Board Certified Pharmacotherapy Specialist (BCPS) or Ambulatory Care Pharmacist (BCACP).

Physical Demands – Job requires sitting for prolonged periods; Repetitive motion: typing. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull

Compensation
The pay rate for this role starts at $64.37 per hour. New hire compensation will be determined by education and experience.

Onboarding Requirements
Background checks and pre-employment health screenings will be conducted on all new hires prior to employment. All safety-sensitive positions with direct patient interaction also require a drug screen.

Position Compensation Range: $55.67 - $89.07 Hourly
