Suncoast Hospice, a member of Empath Health, is seeking an experienced APRN to provide compassionate, high‑quality care to patients living with advanced illness. In this role, the APRN will perform comprehensive assessments, lead advanced care planning, and manage acute and chronic conditions in collaboration with a physician. What you'll Do:
Provide high‑quality APRN care across the Suncoast Hospice service area, including consults and advanced care planning.
Perform comprehensive health assessments, including medical histories and physical examinations, to support accurate clinical decision‑making.
Diagnose, treat, and monitor acute and chronic conditions in collaboration with a supervising physician.
Support patients and families through complex care needs, ensuring comfort, dignity, and alignment with end‑of‑life goals.
Partner with the interdisciplinary team to deliver coordinated, patient‑centered care throughout the hospice journey.
