Then join us! The Trauma Recovery Center team in Arizona is excited to add another Assertive Outreach Case Manager to the team. The Trauma Recovery Center (TRC) serves recent victims of violent crime, including survivors of physical assault, sexual assault, gunshot wounds, stabbings, domestic violence and human trafficking.

LOCATION: 6850 W. Indian School Road

HYBRID REMOTE SCHEDULE!: You'll be able to work hybrid remote, providing telehealth and in-person appointments in the office, field, home or during office hours.

WHAT MAKES YOU A GREAT ASSERTIVE OUTREACH CASE MANAGER?

Your understanding of trauma informed care and behavioral health services continuum of care.
Your awareness and sensitivity to cultural issues and community practices of low-income and at-risk families, including dual language speakers.
Your integrity and reliability, always following through on deadlines and commitments.
Your ability to work with victims of crime, diverse populations, multicultural and Spanish-speaking clients and families.

HOW WILL YOU MAKE AN IMPACT?
As the Assertive Outreach Case Manager, you will provide trauma-informed community outreach to connect victims with Trauma Recovery Center services and work as a member of the interdisciplinary team.

Provide trauma-informed, culturally competent, and client-centered care to diverse, at-risk individuals and families.
Conduct assessments to identify case management needs and develop appropriate service plans.
Connect clients with supportive resources, including financial benefits, Victim of Crime services, disability programs, and immigration-related assistance (e.g., U-Visas, asylum evaluations).
Collaborate closely with clinicians, the Program Manager, and interdisciplinary teams to coordinate and deliver high-quality services.
Advocate for clients by liaising with law enforcement, district attorneys, and other agencies to support reporting, legal processes, and safety needs.
Provide community-based outreach, home visits, and follow-up services to ensure engagement and continuity of care.
Build and maintain strong partnerships with community organizations to expand outreach and awareness of Trauma Recovery Center services.
Maintain accurate documentation in compliance with HIPAA, state/federal regulations, and agency productivity standards, while performing other duties as needed.

MINIMUM QUALIFICATIONS AND COMPETENCIES

Required: High School Diploma or GED with 3 to 5 years of experience providing Case Management services
Preferred: Bachelor's Degree in Communications, Human Services, or related field, or equivalent experience.
Experience within the domestic violence and/or sexual assault field preferred.
Bilingual in Spanish and English required.
Possess a valid driver’s license and ability to be insured under automobile insurance policy.

Other Requirements

Able to work a flexible schedule as needed per program requirements
Able to obtain fingerprint clearance card, CPR and First Aid certificates
Able to work with victims in crisis and be able to offer supportive services
Excellent verbal and written communication skills
Excellent organization and time management skills
Proficiency in computer software systems including Microsoft Office, which includes Excel, Word, PowerPoint, etc.
Successfully complete the agency’s orientation and demonstrate competence regarding skills and knowledge
