Summary:The case management position would be an integral part of the community schools’ team as well as a support to students and families involved with the Tier 1 GCM program. This would be a largely school placed position. Primary responsibility is to work collaboratively with schools, students, and families to address the contributing factors that lead to truancy issues.

Essential Duties and Responsibilities:

  • Adhere to program standards, and the agency’s policies and procedures.
  • Work collaboratively with the community schools' team to create, implement, and monitor student attendance intervention plans.
  • Collaborate with the community schools' team to create and implement incentive plans and events.
  • Attend all school-based partner meetings.
  • Partner with assigned school to meet Tier 1 standards and facilitate school intervention meetings with attendance designee.
  • Follow up weekly with active Tier 1 services to monitor attendance and address contributing factors by linking families to resources, advocating for families at schools, and providing education and information.
  • Complete home visits as needed for Tier 1 referred students.
  • Conduct a thorough family intake and develop a strengths-based family development plan that reflects goals that will address the factors contributing to truancy.
  • Implement the Family Service Plan by locating and linking them to resources, coordinating services and monitoring the family’s use of current and new resources.
  • Obtain monthly school records and collaborate with school staff to address factors contributing to truancy.

· Complete all required documentation Tier 1 service.

· Prepare and maintain all client files per the program standard requirements.

  • Attend bi-weekly supervision on time and be prepared with updated and accurate case files.
  • Adhere to agency attendance policy and attend all required meetings.
  • Attend department and agency events and be available to assist with the planning process.
  • Share weekly schedule/calendar with Program Manager and communicate in a timely manner regarding any changes.
  • Any concern regarding the safety of clients must follow agency and program protocols.
  • Complete 20 hours of ongoing work-related training each contract year.
  • Adhere to agency and professional code of ethics.

Required Education and Experience:

Bachelor’s degree from an accredited program of social work or a comparable human service field with at least two years work experience in the field of family and childrens’ services. Prior home visitation experience, and work with school staff preferred. Valid driver’s license and vehicle to use in job, required. Acceptable Child Abuse Clearance, FBI Clearance, Criminal Investigation Report, and Health Assessment with TB Screening. Familiarity with Microsoft systems and use.

Preferred Education:

Bachelor’s degree from an accredited program of social work or a comparable human service field with at least two years work experience in the field of family and childrens’ services. Work Environment:

This position is a full-time school-based position, employee is required to be onsite at school five days per week. Occasionally travel to the offices of foundation officials or off-site locations will be required. The incumbent in this position will routinely use computers, photocopiers, filing cabinets and other standard office equipment. The position also requires you assist with large gift in-kind drives, and occasional event set up and breakdown.

Physical Demands:

While performing the duties of this job, the employee is frequently required to sit, talk and hear. The employee is occasionally required to walk, use hands and fingers to feel, handle, or operate objects or controls; and reach with hands and arms. The employee must occasionally lift and/or move up to 30 pounds.
