Attendant Care or Respite Care Provider for Teenage Boy

We are looking for an attendant care or respite provider for our 16 year old son on the weekends. He is a sweet, funny boy who loves to swim, jump on the trampoline and go for bike rides.

He is self harming and needs constant supervision. He will need assistance with bathing and self help skills. However, he is learning and is capable of doing things for himself with the correct prompting. There is some snack preparation and cleaning up of the areas he uses.
