Job Description:
As an advocate for the patient, the Behavioral Coordinator assists patients with mental health and substance abuse needs to reach optimal function and health in an appropriate environment that would support their needs. The Behavioral Health Coordinator applies the process of assessment, planning, implementation, monitoring, evaluation and coordination of care to meet the patient's health care needs across the continuum in collaboration with health care team members to ensure safe, quality and cost-effective care. The individual will attempt to resolve social and environmental difficulties interfering with the patients’ ability to maximally manage their mental health and overall quality of life. The Behavioral Health Coordinator will triage referrals from primary care providers to the appropriate behavioral health provider and service. As a member of the multidisciplinary team, the Behavioral Health Coordinator works closely with physicians, financial staff, and ancillary staff to facilitate those functions associated with arranging care and providing referrals. The Behavioral Health Coordinator will possess significant independent judgment, analytical, problem solving, collaborative skills to organize and prioritize multiple tasks to provide patient centered leadership and direction.
