Job Details:The Bereavement Specialist provides emotional support and counseling to family members and significant others of hospice patients following a loss. This role implements bereavement care plans, conducts assessments, and ensures services align with hospice philosophy while supporting families through the grieving process.
Develop and implement bereavement care plans with the interdisciplinary team.
Conduct post-death bereavement assessments and provide grief counseling.
Facilitate bereavement support groups and coordinate follow-up with families.
Provide referrals to community resources and support services.
Assist with memorial services and bereavement education for staff and the community.
Maintain accurate documentation and collect program data.
Master’s degree in Social Work, Counseling, Psychology, Mental Health Nursing, or related field.
Minimum 2 years of clinical experience, preferably in hospice or bereavement care.
Current New York State professional license required.
Knowledge of grief counseling and hospice care principles.
