Bereavement Specialist-PD

The Bereavement Specialist provides emotional support and counseling to family members and significant others of hospice patients following a loss. This role implements bereavement care plans, conducts assessments, and ensures services align with hospice philosophy while supporting families through the grieving process.

Key Responsibilities

• Develop and implement bereavement care plans with the interdisciplinary team.

• Conduct post-death bereavement assessments and provide grief counseling.

• Facilitate bereavement support groups and coordinate follow-up with families.

• Provide referrals to community resources and support services.

• Assist with memorial services and bereavement education for staff and the community.

• Maintain accurate documentation and collect program data.

Qualifications

• Master’s degree in Social Work, Counseling, Psychology, Mental Health Nursing, or related field.

• Minimum 2 years of clinical experience, preferably in hospice or bereavement care.

• Current New York State professional license required.

• Knowledge of grief counseling and hospice care principles.
