Position: Bilingual Case Manager| Full-time | Benefits Eligible | 35 hours per week

PURPOSE
The Long Island Fatherhood Initiative (L.I.F.I) program will address the needs of fathers 18 years & older (most low-income, many with history and involvement in the criminal justice system) in both Nassau & Suffolk Counties. The case manager's primary goal is to work with fathers by providing regular contact to facilitate program retention, completion, and access to needed support services with greater focus on economic stability and job-focused services with a clear and unwavering commitment to promoting responsible parenting and healthy marriage/relationship.
SPECIFIC RESPONSIBILITIES
1. To provide systematic, coordinated outreach, intake, and comprehensive personal assessment services for all eligible participants referred by community-based agencies or identified through extensive outreach efforts
2. Provide case management services to participants via phone, in person and/or virtually. Participants must receive a minimum of 8 case management service contacts
3. Develop a targeted individualized case management plan with clients to identify their short- and long-term goals and assist them with their immediate including coordination of services and support as needed.
4. Provide direct services, including support, education, and focused external referrals to other needed services such as housing, substance abuse treatment, mental health programs, etc
5. Convene weekly with case management team to review caseload, client progress, identify any barriers that could impact program retention/completion or any other programmatic challenges and successes.
6. Intensive focus on economic stability by coordinating needed economic stability supports, identifying obstacles and providing hand-on support to improve their readiness for job seeking and employment including follow-up on participants' progress regarding job placement, retention, and/or training.
7. Work as a part of a team to deliver a range of community outreach and engagement activities to promote services offered through the LIFI program to prospective partner organizations including but not limited to probation/parole, Drug and Family Court, CPS, homeless shelters, transitional and recovery housing facilities
8. Methodically track and maintain records of all client contacts, goal objectives, progress and milestones including employment and education plans in nFORM. Ensure all entries are accurate, detailed and entered in a timely manner.
9. Participate in all professional development opportunities including but not limited to on-going training at conferences and webinars addressing Fatherhood, Healthy Relationships, Domestic Violence and Economic Stability issues.
10. Assist with the facilitation with nFORM surveys if needed
11. Prepare reports as requested and other project duties as assigned including hotline coverage
