We are seeking a bilingual Community Health Worker (CHW) who is passionate about making a difference. In this role, you won’t just have a job—you’ll serve as a trusted advocate, educator, and connector for patients navigating their healthcare journey.

Your Impact:

• Empower patients and families to take control of their health
• Break down barriers to care, including social determinants of health
• Serve as a bridge between the community and healthcare providers
• Build trust within diverse populations through culturally competent care
• Improve health outcomes through education, advocacy, and outreach

What You’ll Do:

• Engage directly with community members through outreach and events
• Provide health education and informal counseling to patients and families
• Help patients navigate healthcare systems and access community resources
• Support care teams with patient follow-ups and coordination
• Maintain accurate documentation in electronic systems
• Advocate for patients’ needs at both individual and community levels

What You Bring:

• Bilingual in English and Spanish (required)
• High School Diploma or equivalent (required)
• Degree in Public Health, Social Work, Psychology, or related field (preferred)
• Community Health Worker Certification (preferred)
• A passion for serving others and improving community health
• Strong communication and relationship-building skills
• Experience working with diverse populations
