Your Role:

• Conduct comprehensive initial assessments for new clients to establish tailored care plans.

• Perform regular reassessments to ensure ongoing quality and effectiveness of care.

• Lead field orientations for caregivers, ensuring they are well-equipped to deliver exceptional service.

• Collaborate closely with our administrative team to maintain compliance and accurate documentation.

• Travel to clients' homes across assigned territories in New Jersey, providing hands-on nursing care and support.

What We’re Looking For:

• Active RN license in the state of New Jersey (Required).

• Active CPR Certification

• Previous experience in home care, hospice, or community health settings is a plus.

• Willingness and ability to travel to various client locations.

• Self-motivated, reliable, and capable of working independently.
