Business Development Manager – Senior Living & Hospitals

Ehsas Home Health is hiring a Business Development Manager – Senior Living & Hospitals to drive referral growth across hospitals and senior living communities in Cook County, Will County, Dupage County and Lake County. The ideal candidate already has strong relationships with discharge planners, case managers, social workers, SNF leaders, and assisted living decision-makers, and knows how to turn those relationships into steady referral volume.

Responsibilities

  • Build and maintain referral relationships with hospital case managers, discharge planners, social workers, care coordinators, rehab directors, and senior living decision-makers.
  • Conduct regular in-person visits to hospitals, SNFs, assisted living communities, and independent living communities within the assigned territory.
  • Present and educate referral partners on Ehsas Home Health service lines, admission criteria, and patient transition processes.
  • Identify new business opportunities and develop a pipeline of qualified referral sources.
  • Track outreach activity, referral conversations, and account progress in CRM.
  • Collaborate with clinical and operational teams to support smooth patient handoffs and strong partner satisfaction.
  • Represent the company at networking events, healthcare meetings, and community outreach activities.
  • Meet monthly and quarterly referral growth goals.

Required Qualifications

  • 3+ years of experience in healthcare business development, referral development, physician relations, hospital liaison work, or post-acute marketing.
  • Existing relationships with referral sources in hospitals, senior living, or post-acute care settings in Cook County, Will County, Dupage County and Lake County.
  • Strong understanding of care transitions, discharge planning, and referral workflow in hospital and senior living environments.
  • Proven ability to grow referrals, build accounts, and manage a territory independently.
  • Excellent communication, presentation, and relationship-building skills.
  • Valid Illinois driver’s license and reliable transportation.

Preferred Qualifications

  • Experience in home health, hospice, therapy, infusion, rehabilitation, or other post-acute services.
  • Familiarity with Medicare, Medicare Advantage, and common post-acute referral pathways.
  • Experience using CRM platforms such as GoHighLevel, Salesforce, or similar systems.

Compensation and Benefits

  • Full-time position.
  • Benefits: PTO, mileage reimbursement, bonus structure, or other benefits offered.
  • Schedule: Monday to Friday 9am to 6pm CST

Pay: $65,739.13 - $82,051.42 per year

Work Location: Hybrid remote in Oakbrook Terrace, IL 60181
