Job Summary
We are a growing home health, IV hydration, and Physical Medicine & Rehabilitation (PM&R) services provider seeking an experienced Business Development Liaison with deep roots in the post-acute and senior living community. This is a relationship-driven role focused on generating patient referrals through your established network of nursing home, assisted living, and independent living communities across Southern Cook County and Will County.
What You'll Do
- Leverage existing relationships with nursing home administrators, social workers, discharge planners, and care coordinators to drive consistent patient referrals
- Conduct regular in-person visits to skilled nursing facilities (SNFs), assisted living communities (ALFs), and independent living communities in your territory
- Educate facility staff and clinical teams on our home health, IV hydration, and PM&R service offerings
- Identify and develop new referral partnerships with hospital case managers, rehab directors, and post-acute care teams
- Represent Jardin/Ehsas at community events, senior living expos, and networking functions
- Maintain accurate records of outreach activity, referral sources, and pipeline in CRM (Go High Level or equivalent)
- Collaborate with clinical leadership to ensure seamless patient transitions and high satisfaction among referral partners
- Meet and exceed monthly referral volume targets
What You Bring
- 3–5+ years of experience as a nursing home administrator, director of marketing, or post-acute business development professional
- Established, active relationships with administrators, social workers, and discharge planners at SNFs, ALFs, and ILFs in Southern Cook County (Orland Park, Tinley Park, Harvey, Calumet City, Oak Lawn area) and Will County (Joliet, Bolingbrook, Plainfield area)
- Deep understanding of the discharge planning process, Medicare/Medicaid eligibility, and post-acute care pathways
- Proven track record of meeting referral or census-building goals
- Self-motivated, organized, and comfortable working independently in a field-based role
- Valid Illinois driver's license and reliable transportation
- Experience with home health, hospice, or therapy services a strong plus
Qualifications
- Proven experience in business development within senior living or post-acute healthcare sectors, with a strong track record of meeting or exceeding sales targets.
- Proficiency in Salesforce, CRM software, and other project management tools to manage client relationships and sales pipelines effectively.
- Exceptional skills in strategic planning, negotiation, and customer relationship management to foster trust and secure partnerships.
- Strong understanding of marketing principles relevant to healthcare services, including digital outreach strategies.
- Excellent communication skills with the ability to present complex ideas clearly and persuasively.
- Demonstrated ability to manage multiple projects efficiently while maintaining attention to detail.
- Knowledge of industry regulations, market dynamics, and key stakeholders within senior care environments is highly desirable.
