About the Role

We are hiring an experienced Physical Therapist for a 13-week contract in Petaluma, CA. This role is in a Skilled Nursing Facility setting and is a great opportunity for a PT who enjoys helping patients improve mobility, restore function, and regain independence.

Responsibilities

• Evaluate patients' physical abilities and develop personalized treatment plans
• Provide therapy to improve strength, mobility, balance, and functional ability
• Educate patients and families on exercises, safe movement, and home strategies
• Monitor patient progress and adjust treatment plans as needed
• Collaborate with nurses, physicians, therapists, and interdisciplinary team members
• Maintain accurate and timely documentation of assessments, progress, and treatment plans
• Ensure care plans follow facility standards and therapy best practices

Requirements

• Active California Physical Therapist license required
• Minimum 1 year of Physical Therapy experience required
• SNF or similar healthcare setting experience preferred
• Bachelor's, Master's, or Doctoral degree in Physical Therapy required
• Knowledge of physical therapy techniques, modalities, and adaptive equipment
• PointClickCare experience preferred
• Strong communication, documentation, and patient care skills
