Summary:
The role of the Nurse Navigator is that of consultant and educator in the implementation and coordination of the multidisciplinary care and education of the cancer patient. The Nurse Navigator is to provide support, education, coordination of care, and other assistance from diagnosis through survivorship for the patient and caregiver. The Nurse Navigator acts as a liaison between oncology services and other ancillary services to ensure the need of patient is being addressed. The Navigator orchestrates timely, effective referral and follow-up communications between referring providers, multidisciplinary specialists and the patient’s support system as well as the community during cancer screening, community forums and support group activities, this is an integral part of the role.
Responsibilities:
Provides individualized coordination of care for the patient and caregiver. Linking both patient and caregivers to community support and financial assistance resources. Navigates the patient from time of diagnosis through completion of treatment into survivorship.
Educates both the patient and the caregiver on the disease process and the treatment options. Assists patient caregiver in using informed decision making process and realistic goal setting in order to empower them to actively participate in their plan of care.
As appropriate, reduces time-to-treatment across multiple treatment modalities, as well as ancillary and support services through effective communication between referring physicians, patients and all cancer care providers. Serves as a liaison for referring physicians and staff. Serves as a resource for physician’s office staff and other clinical staff regarding treatment options, symptom management, community resources and psycho-social issues.
Tracks patient progress along care continuum; anticipates barriers to care and preforms interventions. Acts as patient advocate resolving patient and caregiver concerns. Navigate patient through the potential barriers to ensure their cancer care is in a timely manner.
Participates in marketing and community outreach programs for cancer education, screening and prevention. Partners with community based organizations and providers to provide optimal patient support and survivorship care. Coordinates survivorship events to celebrate then cancer survivor and their caregiver. Provides survivorship care plans at the completion of treatment.

Required
Must be licensed to practice as a Registered Nurse in the State of North Carolina or one of the compact states by the first day of clinical care.
Basic Life Support (BLS) credential
Five years of oncology-related patient care experience
Experience with Microsoft Office or similar desktop productivity tools
Experience leading project-based work
Strong verbal and written communication
Preferred
Care management, disease management or clinical program management experience
Cancer oncology Nurse (CON) certification
