Status: Full Time

Shift: M-F/ no weekends/ no holidays Shift start time between 7am-8am. Shift end time between 12pm-3pm.

Summary:

The Cardiopulmonary Rehabilitation Licensed Practical Nurse (LPN) provides licensed nursing support in both outpatient Cardiopulmonary Rehabilitation and inpatient cardiac rehabilitation settings. Under the supervision of a Registered Nurse and in collaboration with the interdisciplinary care team, the LPN delivers safe, effective, and patient-centered care. Responsibilities include clinical monitoring, documentation, assistance with therapeutic exercise and ambulation, and operational support to ensure continuity of care across rehabilitation settings while adhering to regulatory, accreditation, and organizational standards.

1. Previous Experience Required:

Experience in cardiac care, rehabilitation, telemetry, or acute care preferred.

2. Specialized or Technical Education Required:

Current Licensed Practical Nurse (LPN) license in good standing.

Basic Life Support (BLS) certification required.

Advanced Cardiac Life Support (ACLS) preferred or willingness to obtain.

3. Manual or Physical Skill Required:

Ability to assist patients with ambulation and therapeutic exercise.

Ability to safely operate rehabilitation equipment and monitoring devices.

Responsible for monitoring patient safety during exercise, ambulation, and rehabilitation activities.

Performs assigned duties with accountability while under RN supervision.

Responsibilities:

Clinical Care

Monitor, assess, and document vital signs, symptoms, and patient responses before, during, and after exercise sessions.

Assist with and monitor patients participating in Cardio-Pulmonary Rehabilitation exercise programs.

Perform and document Six-Minute Walk Tests (6MWT) according to departmental protocols.

Assist with ambulation of post-CABG and other cardiac patients in inpatient rehabilitation settings.

Identify changes in patient condition and promptly communicate findings to the supervising RN or provider.

Support emergency response activities in accordance with department policies and scope of practice.

Documentation & Compliance

Accurately document patient assessments, interventions, and responses in the electronic medical record.

Record blood pressures, blood glucose levels, weights, exercise tolerance, and patient education.

Ensure compliance with departmental policies, accreditation requirements, and federal and state regulatory standards.

Operational Support

Support outpatient Cardiopulmonary Rehabilitation operations five days per week.

Assist with inpatient cardiac rehabilitation activities as operationally indicated.

Maintain equipment readiness and adhere to infection prevention and control standards.

Promote efficient patient flow and continuity of care between inpatient and outpatient settings.

Scope & Supervision

Functions within the scope of practice for a Licensed Practical Nurse under the supervision of a Registered Nurse.

Does not independently assess, diagnose, or initiate patient care plans.

Collaborate with Registered Nurses, physicians, therapists, and ancillary staff to support patient care delivery.
