As the Integrated Healthcare Case Manager, you would play a central role in helping our patients access services and take the next steps to wellness. Your work would be primarily in the office with the possibility of some travel to healthcare facilities and patients’ homes. You would be called upon to collaborate with other professionals within and outside our agency to assure that clients receive the best care available.
Coordinate and support patient care activities to promote continuity, access, and quality outcomes across services
Assess and respond to patient needs by addressing clinical, behavioral, and social factors that may impact health and well-being
Facilitate connections to appropriate internal services and community resources to support comprehensive care
Support patient engagement and access to services, helping to ensure timely utilization of care and adherence to treatment plans
Collaborate with interdisciplinary teams and external partners to ensure coordinated, integrated service delivery
Serve as a point of communication between patients, providers, and community organizations to support continuity and alignment of care
Contribute to the achievement of organizational goals, performance metrics, and quality initiatives
Maintain accurate and compliant documentation in accordance with organizational, regulatory, and professional standards
Participate in cross-functional activities and team initiatives to support operational needs and program effectiveness
Assist in the development and improvement of workflows, processes, and best practices to enhance efficiency and service delivery
Identify opportunities for improvement and support problem-solving, innovation, and quality enhancement efforts
Foster effective communication and collaboration across departments to support a team-based approach to care and operations
Perform additional duties as assigned to support program growth, patient care, and organizational priorities
Adapt to change in the workplace and use change as an opportunity for innovation and creativity.
