Shift: Full-Time (40 hours per week on average)
Job Hours: 8:30 PM – 5:00 PM, Monday - Friday with some evening hours required, occasional Saturdays
Base Location: Covington, GA
Division/Department/Program: Residential SUD/GRAN Recovery Center
The GRAN Care Coordinator is responsible for ensuring that clients are connected with the services that they need and for collaborating with staff across the agency and with external agency partners. The Care Coordinator serves as a key administrative, access and operational support role at GRAN. This position provides comprehensive support to the Program Manager, Access team and treatment team. The Care Coordinator will facilitate care coordination for individuals seeking to access care from both internal and external referral sources. The role is crucial for the smooth operation of the program focusing on client support, administrative tasks, and ensuring high-quality care and service delivery.
Under broad supervision, provide a broad range of social services to patients/clients/individuals and their families. Conducts patient/client intakes, participates in treatment team planning, and performs crisis intervention. Assists in dealing with personal and social problems. May perform case management duties.

Minimum Qualifications:
High School diploma/GED and three (3) years of job-related experience in a social services-related position; or One (1) year of experience at the lower-level Social Services Tech 2 (SST011) or equivalent position.
