Care Manager I (Holocaust Survivor)

Job Summary:

We are seeking an experienced Social Worker/Care Manager who will provide community-based services to support holocaust survivors to live quality independent lives in the community. The Care Manager will work with a dynamic team of professionals who provide innovative wrap-around services including, care management, occupational therapy for fall prevention, mental health counseling, caregiver support, volunteer-friendly visitors, and home-based enrichment programs. Care management services are provided to individuals of all income, cultural, and socio-economic backgrounds.

Key Responsibilities:

Specialized Population Focus: Provide culturally sensitive, Russian-speaking care management to elderly Holocaust survivors, tailored to their unique needs.

Care Plans: Develops, implements, and regularly updates person-centered care plans in collaboration with client to support health, safety, and independent living.

Care Coordination: Helping the client navigate, connect to, and communicate with physical & behavioral health and social systems according to their care plan.

In-Person Visits to Clients: Engaging primarily the client in-person or client’s modalities preference to assess their needs, provide support, and ensure the effective delivery of care services.

Support System: Ensuring the client, their family, and their personal support system are educated about their condition and connected to resources needed to support their health goals.

Resources Management: Identifying strategically and optimizing resources to meet the clients’ and their families' unique needs by referring to community and social support services.

Patient Advocacy: Advocate for our clients, safeguarding their rights, preferences, and best interests throughout their care journey.

Communications: Act as the central hub of communication, ensuring a smooth exchange of information among patients, families, healthcare providers, and all involved parties.

Documentation: Maintain detailed and up-to-date records of assessments, care plans, progress notes, and other vital documentation, adhering to legal and organizational standards.

Regulatory Compliance: Stay current on relevant laws, regulations, and policies, ensuring every aspect of care delivery is compliant and up to standard.

Qualifications Education:

  • Bachelor’s degree in social work, Nursing, Public Health, or a related field required

Experience:

  • At least one year working with older adults.
  • Experience with and understanding of the medical and psychosocial problems of functionally impaired older adults.
  • Experience and understanding of the stressors and needs facing caregivers.
  • Experience working with databases, specifically Apricot, is highly desirable.
  • Exceptional engagement skills with older adults, and their support systems.
  • Demonstrates case management skills and experience in community-based settings.
  • Ability to provide trauma-informed support to survivors of trauma.
  • Cultural competence and sensitivity to diverse populations
  • Motivational Interviewing
  • Needs assessment and care planning

Skills and Competencies:

  • Excellent interpersonal and communication skills.
  • Service coordination and navigational skills
  • Strong organizational and time management abilities.
  • Proficiency in data entry and database management.
  • Ability to work independently and as part of a team.
  • Knowledge of the local community and available resources.
  • Bilingual in Russian speaking required

Salary and Benefits:

Salary Range: $64,251 - $73,255 annually. Full-Time, Monday to Friday.

This position is full-time, 37.5 hours/week. The salary is commensurate with the selected candidate's qualifications and experience.
