Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
• Monitor patients’ vital signs and observe their overall health status to identify any changes or concerns promptly.
• Administer medications and follow care plans accurately while maintaining compliance with HIPAA regulations to protect patient privacy.
• Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary.
• Support clients during outings or community activities, fostering social engagement and independence.
• Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning patient areas and laundry.
• Use specialized equipment like Hoyer lifts or resident’s personal assistive devices to ensure safe transfers and mobility assistance.

Experience

• Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred.
• Demonstrated knowledge of patient observation, medication administration, and vital signs monitoring.
• Experience working with individuals with developmental disabilities, Alzheimer’s disease, dementia, or autism spectrum disorder is advantageous.
• Familiarity with care plans, HIPAA compliance, and documentation procedures related to patient care.
• Hands-on experience in providing compassionate senior care or hospice support is a plus.
• Ability to perform heavy lifting safely and effectively when assisting residents with mobility or transfers.
• Strong communication skills for working collaboratively with healthcare teams, social workers, and family members to ensure comprehensive patient support.
