Caregiver Position (PHC)

Job Overview
We are seeking a compassionate and dedicated person to provide high-quality care to patients in their homes and/or facilities. The ideal candidate will have experience in caregiving, particularly for individuals with developmental disabilities, and will be responsible for implementing care plans tailored to each patient's needs. This role requires a strong commitment to patient care, effective communication skills, and the ability to perform physical tasks such as heavy lifting.

Duties

• Deliver personalized care to patients in their home environment.
• Assist with daily living activities, including meal preparation and personal hygiene.
• Implement individualized care plans in collaboration with healthcare professionals.
• Monitor patients' health status and report any changes or concerns to the appropriate personnel.
• Provide support and assistance to individuals with developmental disabilities.
• Perform CPR and other emergency procedures as necessary (If applicable)
• Maintain a clean and safe environment for patients, ensuring adherence to health and safety regulations.
• Document patient progress and maintain accurate records of care provided.

Qualifications

• Experience in home care or caregiving settings is preferred.
• Familiarity with care plans and patient assessment processes.
• Ability to perform heavy lifting as required by patient needs.
• Certification in CPR is not required (we will train you if needed).
• Strong interpersonal skills with the ability to work effectively with patients, families, and healthcare teams.
• Compassionate nature with a commitment to providing high-quality patient care.
• Experience working with individuals in group home settings is a plus.
• Must have reliable transportation and a working smart phone

Job Type: Part-time

Pay: $14.00 - $16.00 per hour
