Job Overview
We are seeking a compassionate and dedicated person to provide high-quality care to patients in their homes and/or facilities. The ideal candidate will have experience in caregiving, particularly for individuals with developmental disabilities, and will be responsible for implementing care plans tailored to each patient's needs. This role requires a strong commitment to patient care, effective communication skills, and the ability to perform physical tasks such as heavy lifting.
Duties
- Deliver personalized care to patients in their home environment.
- Assist with daily living activities, including meal preparation and personal hygiene.
- Implement individualized care plans in collaboration with healthcare professionals.
- Monitor patients' health status and report any changes or concerns to the appropriate personnel.
- Provide support and assistance to individuals with developmental disabilities.
- Perform CPR and other emergency procedures as necessary (If applicable)
- Maintain a clean and safe environment for patients, ensuring adherence to health and safety regulations.
- Document patient progress and maintain accurate records of care provided.
Qualifications
- Experience in home care or caregiving settings is preferred.
- Familiarity with care plans and patient assessment processes.
- Ability to perform heavy lifting as required by patient needs.
- Certification in CPR is not required (we will train you if needed).
- Strong interpersonal skills with the ability to work effectively with patients, families, and healthcare teams.
- Compassionate nature with a commitment to providing high-quality patient care.
- Experience working with individuals in group home settings is a plus.
- Must have reliable transportation and a working smart phone
