Job title: Case Coordinator Team Lead

Purpose/Summary: The Case Coordinator Team Lead is responsible for overseeing a remote team of case coordinators who support the execution of provider orders following telehealth visits in community-based settings. This role ensures timely, accurate, and compliant coordination of care by facilitating order completion, supporting clinical workflows, and maintaining high standards of communication with facilities, providers, and internal teams. The Case Coordinator Team Lead will report to the Community Care Clinical Manager.

The Team Lead serves as both a subject matter expert and a working leader, actively participating in case coordination while providing guidance, oversight, and accountability for team performance

Role & Core Responsibilities:

  • Lead, coach, and support a remote team of case coordinators, ensuring accountability, productivity, and adherence to established workflows
  • Oversee timely and accurate execution of provider orders, including medications, labs, diagnostics, referrals, DME, and home health services
  • Ensure all orders are complete and compliant (ICD-10 codes, patient details, provider credentials, and order specifications) prior to processing
  • Maintain clear, closed-loop documentation with defined actions, status updates, and follow-up on all orders
  • Serve as the escalation point for urgent, complex, or incomplete orders; provide guidance on prioritization and resolution
  • Promote consistent, professional communication across providers, pharmacies, vendors, and internal teams to reduce delays in care
  • Monitor workflow quality and compliance through routine audits; identify gaps and implement process improvements
  • Support operational needs during high-volume periods to ensure continuity of care and timely service delivery
  • Assist with onboarding, training, and ongoing development of team members; reinforce best practices in documentation, communication, and escalation
  • Continuously evaluate team performance and workflow efficiency to drive improved outcomes and turnaround times

Qualifications:

  • Active clinical license required (LPN or higher; RN preferred)
  • Minimum of 2 years of clinical or care coordination experience (SNF, post-acute, or telehealth preferred)
  • Experience with telemedicine platforms and remote care coordination is preferred.
  • Familiarity with electronic health record (EHR) systems and care management platforms is required.
  • Knowledge and experience working with Medicaid and Medicare populations is preferred.
  • Strong critical thinking skills and the ability to handle complex care scenarios is required.
  • Prior leadership and training experience is preferred.

Working conditions:

A designated, secure, and private home workspace is required for this remote role to ensure compliance with HIPAA and company privacy and security policies. This workspace must support confidential patient communications and prevent unauthorized access to protected health information. Typing is required. If a personally owned computer is used for this role, it must meet all company-mandated security and encryption requirements as defined by the IT team. Company-provided security software, including platforms for patient video consultations, may need to be installed to ensure the device remains compliant with organizational data protection standards. Periods of prolonged sitting or standing can be expected.

Pay: $70,000.00 - $78,000.00 per year
