Case Management

Position Summary

We are seeking a compassionate, organized, and motivated Case Manager to join our team at a licensed Substance Abuse and Mental Health treatment facility. The Case Manager plays a vital role in supporting clients throughout their treatment journey by coordinating care, facilitating services, and ensuring continuity of treatment and discharge planning.

This position requires strong documentation skills, the ability to work in a fast-paced clinical environment, and a commitment to client-centered care.

Key Responsibilities

• Conduct intake assessments and assist with treatment planning in collaboration with the clinical team

• Coordinate client services including medical, psychiatric, housing, vocational, and community resources

• Monitor client progress and provide ongoing support and advocacy

• Maintain accurate and timely documentation in accordance with state, federal, and accreditation standards

• Assist with discharge planning and aftercare coordination

• Communicate effectively with families, referral sources, and outside providers (with proper releases)

• Participate in interdisciplinary treatment team meetings

• Ensure compliance with confidentiality and HIPAA regulations

Qualifications

• Experience in substance abuse and/or mental health treatment preferred

• Knowledge of ASAM criteria and levels of care a plus

• Strong organizational and time management skills

• Proficient in electronic health record (EHR) systems

• Excellent written and verbal communication skills

• Ability to work collaboratively within a multidisciplinary team

Preferred Skills

• Experience with discharge planning and community resource coordination

• Crisis intervention skills

• Understanding of co-occurring disorders

• Knowledge of insurance authorizations and utilization review processes
