The purpose of the Case Manager Assistant (CMA) is to support and under supervision of the Clinical Case Manager to coordinate referrals and implement in facilitating the discharge plan developed and identified by the Clinical Case Manager. This position efficiently and effectively coordinating movement through the continuum of care to ensure timely discharge by effectively coordination of necessary durable medical equipment (DME), home care services, home health nurse/aide, therapies, hospice, home infusion, enteral and teaches. The CMA compiles documentation from the patient's chart through Epic to produce a discharge packet for all patients discharging to a SNF/Acute/Rehab facility this packet will be delivered to the patient's floor before the arrival of the ambulance. The CMA monitors all primary and secondary Medicare in-patients insuring they are delivered the Important Message from Medicare before discharge. The CMA researches hospital authorizations approvals/denials and requests for additional medical documentation. Verifies correct level of care in the Clinical Case Management module and Epic Auth Certification tab.

Department Overview
The purpose of the Clinical Case Management Department is to support the interdisciplinary team in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payors.
Department Specific Job Scope
This position works closely with the clinical case managers to coordinate and arrange discharge referrals for home heath, hospice, durable medical equipment (DME), infusion services, enteral services, and bedside teaching. This position is also responsible for delivering chart copies, insurance authorization and denial letters, Important Message (IM) letters, and Commence Health Appeals.
