Case Management Assistant

Job Description

Crittenton Children's Center is seeking a full-time, day shift Case Management Assistant to join our team!

The Case Management Assistant coordinates follow-up care for all hospital patients and residential patients upon request.

  • Monday-Friday, preferred hours are 9-5, buy may consider 8-4

  • No remote work, occasional weekend work needed and may flex days as necessary

  • Schedules aftercare appointments for all hospital patients,

  • Provides additional resources and referrals in addition to appointments,

  • Collaborates with multi-disciplinary team to develop aftercare plan for hospital patients

  • Scribes for daily treatment team meeting for all hospital patients,

  • Works with third party payors to determine follow-up care options

  • Completes 24 hour follow up calls with guardians of discharged hospital patients

  • Completes and mails caring cards for all discharged hospital patients

  • One year of related experience is preferred
