Description

The Department for Aging and Independent Living provides services to aging and physically disabled populations across the state. The Department is committed to assisting individuals in accessing home and community-based services that help prevent premature placement in long-term care facilities. The primary goal of these programs is to support individuals in remaining safely and independently in their homes for as long as possible.

The Case Management Specialist II is responsible for providing comprehensive case management services and oversight of the Personal Care Attendant Program. This role serves individuals with disabilities by offering counseling, advocacy, and case coordination within an assigned geographic area. This position requires frequent travel throughout Eastern Kentucky to conduct in-person visits and support program operations.

Responsibilities and duties include:
  • Provide case management, coordination, and oversight for the Personal Care Attendant Program.
  • Conduct in-home, face-to-face visits with program recipients. This includes regular travel across Eastern Kentucky to meet with recipients in their homes and communities.
  • Assist recipients with completion and submission of required program documentation.
  • Support the development of service objectives, comprehensive service plans, and budgets.
  • Compile and maintain regional data related to recipient demographics.
  • Provide direct assistance to applicants with program applications and recipient assessments.
  • Manage program inquiries and serve as a liaison between recipients and external partners.
  • Oversee the distribution, review, and approval of program applications and assessments in accordance with established policies.
  • Review recipient service plans to ensure alignment with individual needs and approved budgets.
  • Process timesheets and invoices, ensuring compliance with service plans and budget requirements.
  • Interpret policies, regulations, and statutes to ensure program compliance.
  • Facilitate the development of community partnerships with participants, service providers, and employers.
  • Coordinate and monitor provider services to prevent duplication or gaps in care.
  • Develop strategies to enhance community awareness of program services.
  • Participate in assessment review teams and attend meetings as required.
  • Perform other duties as assigned.
Preferred knowledge/skills/abilities:
  • Strong verbal and written communication skills.
  • Proficiency in computer systems and software applications.
  • Ability to plan, organize, and prioritize tasks effectively.
  • Strong attention to detail with a high level of accuracy.
  • Ability to work collaboratively as part of a team.
  • Ability to manage field-based and administrative workloads while meeting program deadlines.

Minimum Requirements
EDUCATION: Graduate of a college or university with a bachelor's degree.


EXPERIENCE, TRAINING, OR SKILLS: One year of experience in social work, counseling, providing direct services to individuals and/or families (i.e., employment services, community action agencies), or experience in reviewing, monitoring, determining eligibility for, or administering the Supplemental Nutrition Assistance Program, Kentucky Transitional Assistance Program, Medical Assistance, Child Support, or other public assistance programs.


Substitute EDUCATION for EXPERIENCE: NONE


Substitute EXPERIENCE for EDUCATION: Experience in social work, counseling, providing direct services to individuals and/or families (i.e., employment services, community action agencies) or experience in reviewing, monitoring, determining eligibility for and/or administering the Supplemental Nutrition Assistance Program, Kentucky Transitional Assistance Program, Medical Assistance, Child Support, or other public assistance programs will substitute for the required education on a year-for-year basis

Working Conditions
Performs job duties in an office setting and visits clients in their homes.
Probationary Period
This job has an initial and promotional probationary period of 6 months, except as provided in KRS 18A.111.
