Position Overview: Family Promise of Greater Cleveland (FPGC) is seeking a skilled and motivated Case Manager Team Lead to support the delivery of high-quality, trauma-informed services for families, with a focus on young parents and young adults (YYA). This role blends direct service, program leadership, and staff support. The Team Lead will carry a caseload of YYA clients, serve as the primary representative for the Youth Homelessness Demonstration Program (YHDP), and provide day-to-day guidance and supervision to Resident Monitors. The Team Lead will also support the Housing and Supportive Services Supervisor in maintaining program quality, consistency, and alignment with FPGC’s trauma-informed framework.
This role requires one shift 12-8 or 1-9 per week (Tuesday and Wednesday rotating).
Principal Duties and Responsibilities:
Direct Service (Primary Function)
- Maintain a caseload of YYA clients, providing trauma-informed, client-centered case management from shelter through community integration
- Develop and implement individualized service plans focused on housing stability, income growth, and long-term self-sufficiency
- Coordinate wraparound services including behavioral health, education, employment, and life skills
- Maintain consistent client engagement through regular meetings and follow-up
- Program Leadership – YYA & YHDP
- Serve as the internal lead for YHDP implementation, ensuring fidelity to program goals and requirements
- Represent FPGC in YHDP-related meetings, collaboratives, and community partnerships
- Track and communicate outcomes, challenges, and trends related to YYA programming
- Support the development and refinement of YYA-focused programming, including ACCESS, employment pathways, and engagement strategies
Team Support & Supervision (Resident Monitors)
- Provide day-to-day supervision, coaching, and support to Resident Monitors
- Reinforce trauma-informed, safety-focused, and relationship-centered practices during evening operations
- Support scheduling coordination, communication, and coverage needs in collaboration with leadership
- Address immediate concerns or incidents and escalate as appropriate
- Program Support & Quality
- Assist the Housing and Supportive Services Supervisor in monitoring service delivery and program consistency
- Support onboarding and informal coaching of case managers, especially related to YYA engagement
- Help identify program gaps, barriers, and opportunities for improvement
- Ensure timely and accurate documentation in client records
- Organizational Responsibilities
- Participate in team meetings, case conferencing, and professional development
- Support special initiatives, events, and agency priorities as needed

Qualifications:
- Bachelor’s degree in social work, human services, or related field (will consider experience in lieu of formal education)
- Minimum three (3) years of experience in case management or a related field.
- Strong understanding of issues related to homelessness, poverty, and family services.
- Skills and Knowledge:
- Knowledge of and interest in human services, especially poverty, homelessness, and unemployment.
- Excellent interpersonal skills, including compassion and sensitivity to social issues.
- Sound judgment and an ability to act quickly and professionally under pressure.
- Ability to work as a team player and support clients and professional staff
Compensation:
- Pay: $23.00 - $25.00 per hour
