Case Manager/Recreation
BASIC FUNCTION:
Responsible for providing, managing, and coordinating all direct clinical services to a caseload of 10 clients and properly overseas and maintaining all clinical records. Organize and lead recreational and educational group activities that support mental, emotional, and physical health.
REPORTS TO:
Clinical Director
RESPONSIBILITIES:
· Assessments: Mental Health/Substance Abuse: to determine the service needs of the caseload to provide proper case management services.
-Service Plans: Twice a year or as needed, based on the assessments.
-Case Management Sessions: 2 per month or as needed
-Substance Abuse sessions: 2 per month or as needed
-Room inspections: 2 a month or as needed
-Recreational Activities: 3 times a week
-Timely Documentation of progress notes
- Referrals and linkage to community resources that meet the diverse needs of tenants.
QUALIFICATIONS:
Bachelor’s degree preferred with two (2) year case management experience.
Or associate degree with four (4) year case management experience or two (2) years college with six (6) years case management and recreational experience.
*Prior experience preferred in case management, crisis intervention and client advocacy. Knowledge of HIV-related intervention and substance issues necessary.
Bilingual English/Spanish a must.

Salary: $37,500.00 per year
