Job Summary
Provide community-based intervention services, brief case management and evidence informed education to
parents and/or expectant parents to decrease the impact of substance use and high-risk factors.
Provide a planned and coordinated educational approach to decrease the effects of substance use within
the family and to increase access to community resources and evidence informed education. Will also make
necessary referrals and coordinate services, by promoting linkages between substance abuse treatment
programs, health clinics and other social service providers to ensure that clients have access to all available
health and social services necessary to obtain an optimum level of functioning. Make frequent contact with
clients in their homes and natural settings to focus on practical problems of daily living, promote assertive
advocacy, assistance with transportation if needed.
Essential Duties and Responsibilities
- Conducts comprehensive assessments, intake, and orientation on program participants to determine individual and family needs.

Assists in the development of service plans based on the identified needs of the client/family.
- Collects necessary data as needed to assist with programmatic evaluation. Maintains accurate, neat, timely, and appropriate documentation of services provided on client file.
- Provides support activities to motivate clients on the caseload to change self- defeating behaviors using Motivational Interviewing and the Stages of Change Model. Support shall include follow- up and support for discharge planning during and following intervention services.

Engages clients and encourages retention in program services to increase completion rates; this may include crisis interventions, encouraging family involvement and providing education.

Makes frequent contact with clients through home visits, telephone calls and office visits in order to encourage and offer support for a positive lifestyle. Identifies family risk factors and helps improve the health and functioning of the family unit by engaging the family members in support and involvement in the intervention/prevention process. Meets with clients and their families on a regular basis to monitor and follow up on their needs and preferences.

Provides case management services in settings that are based on the needs of the clients and the goals of the client's service plan, including office-based, home-based, or community-based locations.

Promotes and encourages entry into substance abuse and/or mental health services and makes referrals, if appropriate, for clients who need formal treatment.

Provides case management activities that enhance the motivation of clients to reduce their risks of reengaging in substance use or risky behaviors.

Works with the treatment team and court personnel to assure clients can access available and essential resources.

Provides alternative activities that focus on child/family bonding and promote a healthy lifestyle.

Provides support for peer led activities.

Provides harm reduction strategies to clients and during outreach activities.

Distributes pregnancy tests and education regarding high-risk behaviors during outreach activities.

Participates in continuous quality improvement (QM) activities as defined and scheduled.
Required/Preferred Education and Experience
- Bachelor's degree in sociology, psychology, social work, or human services.
- Two (2) years of related experience.
