We are seeking a dedicated and compassionate Case Manager to join our dynamic team, committed to empowering individuals and families through personalized support and resource coordination as part of our workforce team. Duties
- Assist and encourage clients to access the social services needed, including, but not limited to, medical care, financial wellness, housing, and access to available community resources.
- Conduct initial comprehensive assessment of clients' needs and support systems and develop a comprehensive, individualized service plan and coordination of the services required to address the identified needs
- Schedule client appointments with partner agencies, clearly communicate details to clients, and document all follow‑up to ensure applications and tasks are completed on time.
- Provide ongoing support to participants after enrollment by monitoring attendance, engagement, and progress, and helping address any challenges that may affect participation.
- Assist participants while they are in class by helping resolve issues related to attendance, scheduling, childcare, transportation, documentation, or other personal circumstances, working closely with instructors and program staff.
- Coordinate referrals to HRA and other public benefit offices, helping clients gather and prepare the paperwork needed to access services.
- Maintain accurate and up‑to‑date case files and participant records to support billing, audits, and reporting requirements.
- Work closely with instructors to track participant progress and step in early when someone needs additional support.
- Build and maintain relationships with community partners to expand the supportive services available to participants.
Requirements
- Bachelor's degree in education, social work, sociology, or related field required. --Candidates who are in the process of completing their degree may be considered, provided appropriate documentation of academic progress and enrollment. --
- Bilingual- Spanish required. Must be fluent speaking, writing, and reading both English and Spanish.
- Minimum of two years of work experience in social services, case management, and related field.
- Experience working in low-income communities, schools, and/or community-based organization (nonprofit).
- Clear and significant experience connecting clients with needed services.
- Attention to cultural competence in all manner of family and community engagement.
- Proficient in Microsoft Office and other online tools.
- Ability to thrive within a fast-paced, results-driven environment and work on multiple projects simultaneously and effectively
- Proven experience in workforce, social work, or related fields .
- Excellent communication skills with the ability to motivate clients through motivational interviewing and empathetic listening.
- Ability to handle sensitive situations with professionalism and maintain confidentiality at all times.
- Strong organizational skills to manage multiple cases efficiently while ensuring thorough documentation of all client interactions.
Job Type: Full-time
Pay: $50,000.00 - $55,000.00 per year
