Case Manager

Job Overview
We are seeking a compassionate, patient, and dedicated Direct Support Professional to join our Residential Treatment Center team and make a meaningful difference in the lives of youth with emotional, behavioral, or developmental challenges. In this vital role, you will help maintain a safe, structured, and therapeutic environment while promoting the overall well-being, personal growth, and life skills development of each resident. You will play an essential part in supporting residents through daily routines, therapeutic activities, and individualized care plans designed to help them reach their highest potential.

Duties

• Assist clients with Activities of Daily Living (ADLs) and meal preparation
• Provide patient care and support in both home and community settings
• Administer medications as prescribed and maintain accurate records according to HIPAA regulations
• Engage clients in meaningful activities that promote social interaction and cognitive stimulation
• Ensure a clean and safe living environment by performing light housekeeping duties
• Collaborate with healthcare professionals to develop and implement individualized care plans
• Maintain a respectful and supportive relationship with clients while promoting their independence
• Maintain accurate reports and documentation of clients activity and behaviors

Experience
The ideal candidate will possess the following skills and experience:

• Familiarity with working with individuals who have developmental disabilities or autism
• Knowledge of first aid and CPR techniques is preferred
• Experience in patient observation and monitoring for changes in behavior or health status
• Understanding of social work principles as they relate to client support
• Experience implementing behavioral intervention plans in a residential or educational setting
