Position Overview:
We are seeking a dedicated and organized Case Manager to join our team. The ideal candidate is detail-oriented, reliable, and committed to providing high-quality support to clients. This position requires strong documentation skills, dependable transportation, and the ability to work on-site in a professional office environment.

Key Responsibilities:

  • Maintain accurate, timely, and professional case notes that are compliant with Ohio Medicaid standards
  • Develop and monitor individualized service plans
  • Coordinate services and referrals to support client needs
  • Transport clients to appointments, meetings, or required services as needed
  • Maintain regular communication with clients and service providers
  • Ensure compliance with agency policies, Ohio Medicaid requirements, and confidentiality standards

Qualifications:

  • Experience creating and maintaining Ohio Medicaid-compliant case notes
  • Valid driver’s license and reliable transportation (required for transporting clients)
  • Ability to work in-office during scheduled business hours
  • Strong organizational and time-management skills
  • Excellent written and verbal communication skills
  • Ability to work independently and as part of a team

Preferred Qualifications:

  • Previous experience in social services, behavioral health, or related field
  • Knowledge of community resources and service coordination
  • Experience utilizing AI tools to support documentation, organization, or workflow efficiency

Compensation Details:
This is a contractor position starting at $21 per hour. Contractors are responsible for their own taxes and benefits.
