Faith In Action Case Manager
The FIA case manager position is part of the FIA social work team, working primarily at our office located in Chelsea, Michigan. In addition, the case manager may act as a liaison for FIA partnerships within the community and work with other staff on our annual client programs. Case managers are expected to take initiative and work independently and in cooperation with other members of the team, meet critical deadlines, and use a high degree of judgment and discretion in the handling of materials and confidential information. This position reports to the Executive Director.
Case Management:
1. Meet with prospective clients in Chelsea and determine eligibility for people seeking assistance, making recommendations where need is apparent and within program guidelines.
· Perform intakes for new and clients recertifying current clients.
· Coordinate casework as necessary.
· Maintain appropriate records for client interactions.
· Determine client needs and how FIA can help meet needs.
2. Provide Supportive Services for clients living in Sharon Ann in Chelsea in partnership with Avalon Housing staff, including the following:
· Provide ongoing, active outreach and creative engagement to on-site tenants.
· Work with tenants and property management to coordinate eviction prevention efforts and develop housing permanency plans.
· Participate in community meetings and manage on-site activities as necessary.
· Maintain appropriate records for client interactions.
3. Negotiate with and advocate for external resources.
4. Coordinate the delivery of services with other area agencies or organizations as appropriate.
Other Responsibilities:
1. Maintain collaborative relationships with other agencies and partners.
2. Participate in appropriate board committees or community meetings.
3. Other case management tasks as needed or assigned.
Required Qualifications
● Education in social work or similar behavioral science or relevant field and/or relevant work experience.

Job Title: Case Manager/Social Worker
Reports to: Executive Director
Location: Chelsea, MI
Status: Full time position, salary, exempt $49,000 to $51,000 annually.
Schedule: This full-time position is 40 hours per week, with occasional evening hours and infrequent weekend hours.
Benefits: 3% retirement match, 3 weeks of vacation plus additional holidays, 1 week of sick time; medical and dental benefits.
