SUMMARY STATEMENT
:
The Case Manager position is essential in supporting individuals aged 18+ who are transitioning from incarceration back into society. Reporting to the President, Vice President of Programs, and JCM Program Manager, the case manager will provide direct services, outreach, case management, cultural mediation, and advocacy to help participants access critical services and regain control of their health and lives. The case manager will work closely with individuals to navigate complex systems and build a better future, with the ultimate goal of reducing recidivism and promoting recovery. This role is perfect for someone with lived experience, a strong connection to the community, and a passion for supporting others in their reintegration and success. The position requires a high level of independence, selfmotivation, and the ability to effectively engage with participants from diverse backgrounds within their communities.
Duties And Responsibilities

Caseload: Manage a caseload of 30 participants, providing individualized support and case management, while ensuring the timely completion of all required documentation.

Advocacy and System Navigation: Provide advocacy services for participants, including attending court dates, DCFS meetings, doctor appointments, etc.

Documentation: Maintain accurate and organized participant files, documenting activities, progress, and other contract-mandated information including using multiple case management data tracking systems.

Reporting: Sharing program data through regular meetings, check-ins, and assessments.

Community Engagement: Helping participants connect with community services and activities.

Care Plans & Assessments: Coordinate supportive services for clients, ensuring comprehensive and timely follow-up. Provide referrals for participants to extended community-based services and maintain strategies for ongoing resource connections. Conduct extensive client assessments for the purpose of setting goals and tracking progress.

Team Participation: Actively contributing to team-building activities within the CIS and RICMS programs, as well as participating in required events during both weekdays and weekends. Represent JCM at community events such as health fairs, forums, and other public functions to increase awareness and engagement.

Transportation: be willing to transport clients and use personal vehicle for work purposes.
Qualifications & Experience

Lived Experience: Formerly incarcerated or criminal justice system involved preferred.

Education & Experience: High school diploma or equivalent experience in case management, social work, or a similar role.

Legal Eligibility: Permanent resident or US citizen with proof of work eligibility. Criminal background check is required for work with children and adolescents.

Driver’s License and Insurance: Must have valid California Class C Driver's License, minimum automobile insurance, reliable transportation, and be willing to transport clients and use personal vehicle for work purposes.

Adaptability and Professionalism: Ability to work independently in a community-based environment, while managing a caseload and meeting program requirements.

Proficient in Computer and Data Systems: Experienced with utilizing case management software to track and document client interactions, progress and outcomes. Comfortable with basic computer operations including data entry and general software navigation.

Bilingual in Spanish is preferred.

Successful completion of a TB test upon hire.
