CASE MANAGER
Reports To: General Assistance Director, Calumet Township

Job Summary
To complete initial application process, then interview, investigate and determine the extent of eligibility for all persons applying for township assistance, recommend the type and amount of assistance to be used based on the township financial and non-financial guidelines, seek other potential funding sources and make referrals for township assistance applicants and/or recipients who are eligible for assistance from other agencies.
Essential Duties & Responsibilities
- Must be punctual and have good attendance.
- Issue applications (Intake/Recertifications) for persons requesting township assistance, interview all applicants, complete all forms if necessary (Intake, recertify active cases), determine and verify their financial eligibility by completing and/or receiving the verification from wage earners, bank forms, unemployment forms, supplemental unemployment benefits (SUB), Social Security forms, Veteran’s Administration forms, child support forms, check stubs, referrals from the Indiana Family and Social Services, statements from place of employment, verification of contributions (notarized statements).
- Interview all applicants and determine and verify their non-financial eligibility by verifying application (180-day recertification), progress report, workfare compliance or exemption verification, residence compliance and receipts (past 30 days), expenses or verification of need.
- Interview all applicants and determine and verify eligibility for specific assistance requested by completing and/or obtaining rental allowance forms, building inspection forms, Statement of Goods and Services (funeral expense statement) and a variety of other forms when necessary.
- Issue eligible township assistance requested by completing statistical (stat) card, rent/mortgage cards, replacement purchase affidavit (medical/hospital) and maintain all township assistance relief cases in proper sequence and complete the required forms to maintain case control.
- Counsel and make necessary referrals to applicants to the proper community service/assistance agencies.
- Complete daily worksheets and household status worksheet.
- Assist clients in compliance with current policies and procedures, Standards of Eligibility, and current state statues.
- Issue Notice of Township Relief Action (TA-1A) on those cases that require such action.
- Relieve supervision of minor and routine operational details.
- Maintain security of confidential information. o Prepare cases for destruction (i.e. breakdown, log, etc.)
- Have knowledge and be compliant with employee policies and procedures.
- Perform other duties as assigned by the Trustee, Chief of Staff or General Assistance Director.
Education/Training/Experience/Required Qualifications
- High School Diploma or G.E.D.
- Basic knowledge of office procedures.
- Knowledge of basic computer skills and computer applications.
- One (1) to two (2) years of experience in clerical, investigations, and/or social work. o Certifications and/or licensing are not required for this position but are a plus if had.
Skills and Abilities
- Ability to maintain and respect confidentiality.
- Knowledge of township assistance procedures.
- Ability to self-direct work.
- Comprehension of laws pertaining to township assistance (Indiana Code, Title 12, Article 20).
- Ability to make decisions in accordance with laws, ordinances, regulations and departmental policies and procedures.
- Ability to interpret rules, regulations policies and to make decisions in accordance with established precedent.
- Ability to communicate orally and in writing.
- Effective stress coping skills.
- Ability to be patient with applicant/client and circumstances.
- Ability to respect cultural, social, and racial differences.
Pay: $40,000.00 - $45,000.00 per year
