Job Summary
Responsible for reaching out to individuals residing in vehicles on streets focusing on encouraging eligible households to join the County of Santa Clara’s Community Housing queue and complete assessments to help households get onto the pathway to secure stable housing. Within the City of Mountain View the Homeless Outreach Case Managers works in coordination with the City’s Police Department - Neighborhood and Event Services (NES) team. As a part of the case management, help ensure clients are accessing safety-net services including support applying or renewing eligibility for public benefits such as CalFresh, Social Security Disability Insurance, Supplemental Security Insurance (SSI) and information and assistance to supportive services to ensure housing stability.

Duties

  • Provide case management to individuals and families.
  • Conduct intake assessments and periodic reassessments.
  • Develop and monitor individualized case plans with measurable goals.
  • Assist clients in accessing public benefits (CalFresh, GA, SSI, SSDI) and community resources including employment, mental health, and legal services.
  • Support housing search efforts, housing applications, landlord communication, and housing retention.
  • Maintain accurate and timely documentation in Salesforce and Clarity (HMIS) databases.
  • Ensure compliance with program requirements and maintain program statistics.
  • Conduct outreach as needed, including occasional evenings and weekends.
  • Participate in agency programs and events.
  • Perform related duties and special projects as assigned.

Homeless Services Case Manager

  • Carry a caseload of un-housed individuals and families requiring intensive support.
  • Complete vulnerability assessments (e.g., VI-SPDAT) and assist clients in accessing the County Community Housing Queue.
  • Enroll and manage clients in UPLIFT (bus pass program).
  • Identify and address barriers to supportive housing placement.
  • Coordinate access to shelter, transitional housing, and permanent housing placements.
  • Provide ongoing stabilization support post-placement.

Vehicle Outreach Case Manager

  • Conduct regular field-based outreach to individuals residing in vehicles.
  • Build rapport and provide engagement-focused case management.
  • Complete vulnerability assessments (e.g., VI-SPDAT) and assist clients in accessing the County Community Housing Queue.
  • Identify and address barriers to supportive housing placement.
  • Conduct follow-up visits to support housing stability.

Homeless Prevention Services Case Manager

  • Maintain a caseload of households at risk of losing housing.
  • Conduct risk assessments and coordinate rental and utility assistance.
  • Provide budgeting guidance and stabilization planning.
  • Collaborate with landlords and service providers to prevent eviction and resolve housing crises.

Skills & Competencies

  • Strong interpersonal, listening, and conflict resolution skills.
  • Excellent written and verbal communication skills.
  • Ability to manage multiple priorities, meet deadlines, and maintain professional demeanor under pressure.
  • Ability to exercise sound judgment, initiative, and independence in daily tasks.
  • Proficiency in Microsoft Office (Word, Excel, PowerPoint) and ability to learn database platforms.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, or related field preferred; lived experience and/or significant relevant work experience may substitute for formal education.
  • Minimum of two years’ experience providing case management to low-income or vulnerable populations.
  • Knowledge of local community resources and public benefit systems.
  • Experience working with diverse or historically marginalized communities.
  • Bilingual English/Spanish required.
  • Valid California Driver’s License, reliable vehicle, and current auto insurance.
  • Must pass background checks including criminal record, motor vehicle, and credit history.
  • Willingness to work flexible hours, including some evenings and weekends.

Work environment and Physical Demands:

  • Work is performed primarily in an office environment and the employee in this position is subject to inside environment conditions.
  • May be required to lift up to 20 lbs.
  • Required sitting 60% of time
  • Requires work on computer 60% of time
  • Monthly outreach work, which requires standing and walking primarily outside
  • Travel by car for this position includes travel independently or as part of a small group 10% of a business week, during some periods. Also, travel by car may include attending meetings representing the agency.

Job Type: Full-time

Pay: $29.00 - $30.50 per hour

Expected hours: 40 per week
