Position Summary:
The Individual Supports Case Manager is responsible for developing, coordinating, and monitoring individualized supports for adults with developmental disabilities who live independently or in community-based settings. This role provides skill training, supportive counseling, case management, and crisis intervention while promoting personal choice, independence, and community inclusion. The Case Manager ensures all services align with person-centered planning principles, best practices, and applicable regulatory standards.

Key Responsibilities:

Provide direct skill training, case management, supportive counseling, and crisis intervention as needed.

Facilitate opportunities for individuals to increase independence in areas such as decision-making, community membership, daily living skills, relationships, and personal accomplishments.

Conduct regular home and community visits to assess safety, well-being, and satisfaction of individuals served.

Participate in the planning, development, and review of Individual Support Plans (ISPs) to ensure appropriate supports in health, safety, and personal goals.

Coordinate access to community services, resources, and supports to promote inclusion and independence.

Provide transportation to appointments, activities, and meetings as needed.

Collaborate with healthcare providers, community agencies, and other stakeholders to ensure coordinated and effective services.

Maintain accurate and timely documentation of all services and activities in compliance with agency policies, funding sources, and regulatory requirements.

Support individuals in developing and maintaining social connections, employment opportunities, and recreational activities.

Attend team meetings, trainings, and professional development activities.

Perform other job-related responsibilities as assigned.
Required Qualifications:

Minimum of 2 years’ experience in case management, direct support, or a related human services role.

Knowledge of person-centered planning, developmental disabilities, and community resources.

Strong organizational, documentation, and communication skills.

Ability to work independently while maintaining collaborative relationships with teams, caregivers, and community partners.

CPR/First Aid Certified (or ability to obtain certification within designated timeframe).

Valid driver’s license, reliable transportation, and willingness to travel extensively.
Preferred Qualifications:

Bachelor’s degree in Human Services, Social Work, Psychology, or a related field.

Experience in Shared Living, Adult Foster Care, or home-based support models.

Experience supporting individuals living independently or in community-based settings.

Familiarity with Medicaid, MassHealth, and related funding sources.

Knowledge of Electronic Health Record (EHR) systems.

Bilingual or multilingual abilities.
Working Conditions & Physical Requirements:

Travel: Frequent travel to client homes, community locations, and meetings. Travel may include navigating urban, suburban, and rural areas, with occasional exposure to varying weather conditions. Prolonged periods of driving may be required.

Mobility & Activity: Ability to enter and work in a variety of home environments, including multi-story homes, apartments, and community spaces with physical or environmental challenges (stairs, limited accessibility, pets, or varying temperatures). Extended periods of walking, standing, bending, reaching, and navigating different environments are required. Physical needs may fluctuate based on the individual and the setting.

Lifting & Carrying: Occasional lifting, pushing, or pulling of up to 25 pounds to assist individuals, move program-related materials, or set up equipment.

Computer & Documentation: Frequent use of computers, tablets, and other technology for documentation, case notes, and communication. Extended periods of sitting may be required.

Physical Assistance & Crisis Support: May be required to provide hands-on assistance to individuals with mobility needs or during crisis situations, following safety and trauma-informed practices.

Schedule Flexibility: Ability to work varied hours, including occasional evenings and weekends.

Environmental Conditions: Work occurs in a combination of indoor and outdoor settings, including homes, community spaces, and program sites. May involve exposure to moderate noise levels, varying temperatures, or other environmental factors.

Communication: Strong verbal and written communication skills are required to interact effectively with clients, families, staff, and community partners.

Emotional Demands: Regular exposure to emotionally sensitive or high-stress situations, including client crises, behavioral challenges, and family dynamics. Requires emotional resilience, empathy, and professionalism while remaining calm and solution-focused.
