Title: Case Manager / PIO Coach

Case Manager / PIO Coach
This position provides case management services to program participants as well as teaching the Parenting Inside-Out class series.
Primary Responsibilities
· Case management
o Complete intakes, strengths and needs assessments, and create individualized case plan for participants.
o Meet with each participant at various locations a minimum of eight times during the program to set goals and provide supportive services based on the participant’s case plan.
o Ensure components of economic stability and employment opportunities are offered to each participant.
o Provide referrals as needed.
o Based on needs identified in the assessment, complete activities focused on economic stability and employment activities with a minimum of 3 related to economic stability.
o Track Participant goals, objectives, progress, milestones, and attendance in the case management system.
· PIO Coach
o Visit each class site to ensure that it is adequate.
o Host two cohorts per year with a minimum of 20-25 participants per cohort.
o Conduct engaging and interactive parent and healthy relationship education using the evidence-based PIO curricula.
o Provide make-up lessons for participants who miss a workshop.
o Ensure that simple meals are provided for parents and children in each class.
o Set up and take down for each class at the class site.
o Collect and evaluate workshop results.
Education / Experience
· A high school diploma or equivalent.
· Have a minimum of one year of experience working with at-risk adult populations.
· Have a minimum of one year of experience making in-person presentations to small and large groups.

This position requires the use of a personal vehicle and a minimum level of insurance coverage for that vehicle.
:
· Personal liability insurance with $50,000 for bodily injury per person, with an aggregate of $100,000 per accident, or
· $100,000 combined single limit (bodily injury and property damage combined)
