Job Summary

We are seeking a compassionate and dedicated Case Manager to provide comprehensive support and advocacy for youth in our residential facility. You will be responsible for managing a caseload of residents, developing individualized service plans, and coordinating with families, medical and behavioral health professionals, and community resources to ensure a safe and therapeutic environment.

Key Responsibilities

Individualized Care Planning: Conduct intake assessments and develop goal-oriented service plans tailored to each youth's emotional, social, and educational needs.

Case Coordination: Act as the primary liaison between the resident, their family, legal parties, and external service providers (e.g., therapists, schools,case manager or CASA).

Progress Monitoring: Conduct regular one-on-one meetings with residents to evaluate progress toward goals and adjust care plans as necessary.

Documentation: Maintain accurate, timely, and confidential case files in compliance with state licensing and HIPAA standards.

Advocacy & Referrals: Link youth to essential community resources, including mental health services, vocational training, and legal aid.

Crisis Intervention: Respond calmly and effectively to behavioral crises, utilizing Trauma-Informed Care (TIC) or TBRI principles.

Qualifications & Skills

Education: Bachelor’s degree in Social Work, Psychology, Sociology, or a related human services field is required (or equivalent experience.)

Experience: Minimum of 1–2 years of experience working with at-risk youth or in a residential setting.

Certifications: Must be CPR/First Aid certified (or willing to obtain).

Communication: Exceptional verbal and written communication skills for reporting and interdisciplinary collaboration.

Requirements: Must be at least 21 years of age, pass a criminal background check, and possess a valid driver’s license.

Job Type: Full-time

Pay: $22.00 - $26.00 per hour

Expected hours: 40 per week
