Job Overview
We are seeking a highly skilled and compassionate Case Manager to join our team. The ideal candidate will be responsible for providing supportive services to individuals and families, assisting them in navigating the complexities of healthcare and social services, and ensuring they receive the resources they need to achieve their goals. The successful Case Manager will possess a deep understanding of behavioral health, crisis intervention, and workforce development, with a focus on motivating clients to make positive lifestyle changes.
Responsibilities
- Provide individual and group counseling services to clients, utilizing evidence-based practices such as motivational interviewing and behavioral health techniques to address addiction and related issues.
- Develop and implement comprehensive treatment plans that address clients' physical, emotional, and behavioral needs.
- Collaborate with other healthcare providers and social services to ensure a multidisciplinary approach to client care.
- Conduct home visits, outreach, and community-based services to engage clients and provide support in their natural environments.
- Conduct crisis intervention and manage crisis situations as needed.
- Provide guidance and support to clients as they navigate the Section 8 housing program and other social services.
- Mentor and supervise other case managers, providing feedback and coaching to help them grow professionally.
- Ensure clients are aware of their Medicare benefits and assist them in accessing these services.
- Monitor client progress and adjust treatment plans as needed to ensure clients are meeting their goals.
- Maintain accurate and timely records of client interactions, services provided, and outcomes achieved.
Qualifications
- Proven experience in social work, behavioral health, or related fields with a focus on crisis intervention and crisis management.
- Strong knowledge of addiction counseling techniques and behavioral health best practices.
- Familiarity with child welfare systems, Section 8 housing programs, Medicare services, and workforce development initiatives.
- Excellent communication skills with the ability to motivate clients through motivational interviewing methods.
- Supervising experience is preferred for leadership roles within the team.
- Demonstrated ability to handle sensitive situations involving mental health crises or child safety concerns.

Pay: $42,000.00 - $45,000.00 per year
