Duties
- Conduct comprehensive biopsychosocial and functional needs assessments under Targeted Case Management (TCM) guidelines to evaluate behavioral health status, substance use history, medical needs, housing stability, and eligibility for community and social service supports.
- Develop and implement individualized, person-centered service plans that align with clients’ recovery goals, addressing substance use treatment, mental health stabilization, and social determinants of health.
- Coordinate and monitor care across multidisciplinary providers, including behavioral health clinicians, primary care, probation/parole officers, housing programs, and community-based organizations to ensure continuity of services.
- Provide crisis intervention and stabilization support for individuals experiencing acute mental health symptoms, relapse risk, or behavioral crises, ensuring appropriate referrals and safety planning when necessary.
- Utilize evidence-based engagement strategies, including Motivational Interviewing (MI) and trauma-informed approaches, to enhance client participation, reduce ambivalence toward recovery, and promote long-term behavioral change.
- Deliver ongoing case monitoring, mentoring, and recovery support to reinforce treatment adherence, appointment compliance, and progress toward employment, education, housing, and wellness goals.
- Advocate on behalf of clients to remove barriers to care, facilitating access to substance use treatment programs, psychiatric services, workforce development initiatives, childcare assistance, transportation, and other essential resources.
- Maintain accurate, timely, and compliant documentation of assessments, service plans, referrals, case notes, and progress reports in accordance with agency policies, state regulations, HIPAA, and confidentiality standards.
Requirements
- Masters degree in social work, behavioral health, or related fields.
- Strong skills in crisis intervention, crisis management, addiction counseling, and behavioral health support.
- Knowledge of welfare systems, housing programs, DES & Medicaid processes, and workforce development initiatives.
- Excellent communication skills with the ability to motivate and build rapport with diverse populations.
- Ability to handle sensitive situations with professionalism and transparency while maintaining client confidentiality.
- Demonstrated competency in mentoring or supervising team members or peers is a plus.
- Education in social work, psychology, counseling or related fields is highly desirable; relevant certifications such as Motivational Interviewing are advantageous.
