JOB SUMMARY:

The Angel Arms/Healing Hearts Case Manager provides housing-focused, HUD-compliant case management services to individuals and families participating in Permanent Housing programs. This position emphasizes housing stability, income growth, connection to mainstream benefits, and ongoing monitoring of client progress in accordance with HUD, VA, and Continuum of Care (CoC) requirements. The Case Manager supports program performance outcomes, including housing retention, income increases, and successful exits, while maintaining accurate documentation and data integrity.

ESSENTIAL FUNCTION:

  • Provide ongoing, housing-focused case management services for Angel Arms Singles & Families and Healing Hearts Permanent Housing participants in alignment with HUD best practices.
  • Collaborate with program leadership and intake staff regarding new client placements and transition plans following program enrollment.
  • Develop, implement, and regularly update individualized Housing Stability Plans in partnership with clients, focusing on housing retention, income, benefits, and self-sufficiency goals.
  • Conduct regular client meetings to monitor progress toward goals, identify barriers, and adjust service plans as needed, consistent with HUD monitoring expectations.
  • Ensure compliance with HUD, VA, and CoC regulations, performance benchmarks, and documentation standards.
  • Maintain appropriate follow-up with exited clients, in coordination with the Social Service Manager, to support housing retention and outcome tracking.
  • Maintain consistent communication with the Outreach Director regarding client progress, risks to housing stability, and intervention needs.
  • Provide employment and income-related support as appropriate, including job readiness assistance, referrals to workforce programs, and connection to earned and non-cash benefits.
  • Assist clients in establishing and maintaining personal savings plans and budgeting strategies to promote long-term housing stability.
  • Conduct apartment inspections as needed to monitor unit habitability, safety, and lease compliance, documenting findings in accordance with program requirements.
  • Maintain complete, accurate, and timely case notes, service plans, and client files in compliance with HUD documentation and record retention standards.
  • Enter and maintain required client data in HMIS or other designated data systems to support monitoring, reporting, and performance measurement.
  • Participate in internal file reviews, monitoring visits, and corrective action processes as required.
  • Participate in trainings related to HUD regulations, trauma-informed care, housing first principles, and best practices at the request of the Outreach Director.
  • Participate in the on-call rotation as scheduled to respond to client or housing-related concerns.
  • Refer clients to appropriate local, state, and federal mainstream resources (e.g., SSI/SSDI, SNAP, Medicaid, VA services) based on assessed needs.
  • Transport clients to appointments, classes, inspections, or services as needed, in accordance with agency transportation policies.
  • Serve as a liaison between St. Vincent de Paul, residents, landlords, and community partners to support housing stability and program compliance.
  • Perform other duties as assigned to support program effectiveness and compliance.

  • Standard office equipment, including printers, copiers, and fax machines

WORKING CONDITIONS:

  • Not substantially exposed to adverse environmental conditions.

MINIMUM QUALIFICATIONS:

  • Bachelor’s degree or equivalent experience
  • Computer proficiency with Microsoft Office and data management systems
  • Knowledge of HUD-funded housing programs and service documentation standards preferred
  • Strong organizational, interpersonal, and communication skills
  • Ability to work independently and collaboratively as part of a multidisciplinary team
  • Positive, client-centered, trauma-informed, and housing-focused approach

Pay: $19.00 per hour
