Case Manager

Job Overview:
We are seeking a dedicated Case Manager to join our team. As a Case Manager, you will play a crucial role in providing support and guidance to individuals in need. Your primary responsibility will be to assess clients' needs, develop care plans, and coordinate services to ensure their well-being and progress.

Responsibilities:
- Conduct assessments and interviews with clients to identify their needs and develop personalized care plans.
- Provide crisis intervention and support to individuals facing behavioral health challenges.
- Collaborate with healthcare professionals, social workers, and other stakeholders to coordinate services for clients.
- Monitor clients' progress and help to adjust care plans as needed to ensure optimal outcomes.
- Advocate for clients' rights and access to necessary resources.
- Document all interactions, assessments, and progress notes accurately and timely.
- Participate in case conferences and meetings to review client cases and develop strategies for improvement.
- Stay updated on relevant policies, procedures, and regulations in the field of social work and healthcare.
