At Victory House of the Lehigh Valley, we’re more than just a shelter—we’re a community that helps men experiencing homelessness rebuild their lives with dignity, stability, and hope. We’re seeking a compassionate and organized Case Manager to support residents participating in our transitional housing program.

This role is ideal for someone who enjoys building relationships, coordinating services, and helping individuals move from crisis toward independence in a structured, short-term residential setting.

What You’ll Do

· Complete intakes, assessments, and goal planning with newly admitted residents

· Develop and manage individualized case plans focused on housing stability, income, health, and life skills

· Meet regularly with residents to track progress, address barriers, and update goals

· Connect residents to community resources including housing, benefits, healthcare, employment, and behavioral health services

· Support residents in preparing for successful transitions out of shelter, including housing placement and aftercare planning

· Advocate for residents and coordinate services with partner agencies and providers

· Maintain accurate, timely, and professional case documentation and reports

· Collaborate with residential staff, program leadership, and other team members to support resident success

· Create a respectful, trauma-informed, and supportive environment for all residents

What We’re Looking For

· Bachelor’s degree in Social Work, Human Services, Counseling, or a related field

· Experience providing case management or supportive services, preferably in shelter, housing, or community-based settings

· Strong organizational, communication, and documentation skills

· Ability to manage multiple cases in a fast-paced environment

· Compassionate, professional approach to working with individuals experiencing homelessness

· Comfortable working as part of a team while also managing independent responsibilities

· Basic computer skills (Word, email, databases)

Additional Information

· This position reports to the Director of Programs and Services

· Must successfully pass a drug screen and background check

· Some evening or weekend hours may be required based on program needs

· Staff are expected to maintain confidentiality, uphold agency policies, and model the values of Victory House

Pay: $42,000.00 - $49,000.00 per year
