Overview
We are seeking a dedicated and compassionate Case Manager to join our team at Open Door Ministries- Arthur Cassell Transitional House. The ideal candidate will play a crucial role in supporting veterans by providing guidance, resources, and advocacy. This position requires a strong commitment to improving the lives of clients through effective case management practices and collaboration with community resources.
Overall Responsibility
The case manager is responsible for case files concerning the clients in the Grant Per Diem programs. The case manager will focus on the needs of program residents, make effective referrals as needed and be responsible for the general care of the facility.
Internal Duties
- Uphold and strengthen the purpose and mission of Open Door Ministries.
- To follow and implement all aspects of Open Door Ministries Inc. policies and procedures manual.
- Communication of resident issues with Clinical Director.
- Development of resident treatment plans and updates in note form as required by funding sources.
- Maintain confidentiality of all information concerning program residents.
- To meet individually with the Grant Per Diem program residents on a bi-weekly basis.
- To engage in client updates with Clinical Director and VeteransAdministrationCase Managers.
- To always conduct oneself in a professional and ethical manner.
- Observe the dress code (Business Casual) Daily.
- Resolve internal program issues with residents.
- Implement drug testing as necessary to program residents as required by funding sources.
- Monitor food supplies and educate residents on proper shopping behaviors and life skills associated with independent living.
- Make effective and necessary referrals to local agencies assisting residents to achieve treatment plan goals.
- Take active role in implementing program curriculum which focuses on life skill straining through individual and group sessions.
- Develop effective communication and working skills with all staff to build an effective treatment team.
Direct Services, Support, Development
- Provide referrals for transportation to and from appointments in company vehicle.
- Prepares and documents thoroughly, all significant events via incident reports.
- Performs all related duties as required or assigned by Clinical Director.
- Documentation of information in the HMIS system, along with appropriate documentation in paper files of required paperwork by the funding source.
- Attend all staff meetings for Arthur Cassell Transitional House and Open Door Ministries.
- Attend special events that program participants attend.
- Demonstrate to resident the appropriate Role Modeling for residents to follow.
- Guide and assist interns who are referred by local colleges and universities.
Community involvement
- Develop appropriate communication with community organizations,.
- Utilize appropriate communication skills with program participants, individuals requesting entrance into ODM program and all individuals associate with ODM.
- Continue to update education in order to better deliver services to program participants.
- Research and develop appropriate connections with agencies and service providers in the community.
Qualifications
Candidates should possess the following skills and experience:
- Bachelor degree in Social Work or another Human Service Field
- 1 year experience in related field or practical job experience.
- Certification in CPR/ First Aid training
- Current/ Valid NC Driver's License.
- Excellent communication and organizational skills.
- Ability to motivate people,
- Ability and desire to work as a team with staff, interns and volunteers.
- Ability to problem solve effectively with individuals.
- Sensitive to the needs and issues experienced by the homeless veteran populations.
